Request account deletion
You can request deletion of your Rx ReLeaf patient-app account and associated data. To protect you, our privacy team will verify the request using the email or mobile number already connected to your account.

- We verify that you own the account.
- We delete the app account and other data that is eligible for deletion.
- We confirm when the request is complete.
Healthcare, payment, fraud-prevention, and legal records may need to be retained for the period required by law. Requesting app-account deletion does not cancel appointments or erase records that Rx ReLeaf is legally required to keep.
Do not include diagnoses, treatment details, identification documents, or other medical information in ordinary email.
